Yes indeed, they would provide a great addition gameplay-wise, i would also remove the more retarded money gathering methods like exploring for collectibles or milk making, since in my view the game about british shithole rapetown should be more realistic and harder than what we have.
The biggest change (in my vision) would probably giving prostitution a bigger role, mayhaps even make it the biggest paying option of all. And here the STDs come into play, since obviously getting infected would lower your prices you would want to avoid them as much as possible.
And of the off chance you do get them your player character would have to persuade their clients to use their services, adding another layer of sensuality on top, since the dynamics would shift tremendously.
And as you mention of course condoms would also play a bigger role in the game too. I could go on and on about gameplay changes, but lets leave it at that for now.

Anyways they had STDs in LonaRPG (the peak of broken rape victim simulators) and it wasnt that bad visually-wise, and im pretty sure you could cure them easily anyway.
